Maternal-Fetal Nutrition during Pregnancy and Lactation

Delivery of an optimally grown, viable infant defines a successful pregnancy. Optimal growth is achieved by the interaction of maternal, placental, and fetal systems to deliver maternal nutrients to the placenta, transfer them to the fetus, and maximize their utilization for fetalgrowth. Pregnancy is characterized by profound changes in the maternal immune, metabolic, cardiovascular, and renal systems to ensure a asuccessful pregnancy and adequate fetal growth. The fetal-placental unit secretes many hormonal signals, the roles of which include redirecting maternal physiology and metabolism to direct substrate toward the fetus and support normal fetal growth. The physiological adaptations of pregnancy begin shortly after conception, indeed before the establisment of a fetal placental unit, and thus in their early phases must be directed by maternals signals, including those from the corpus luteum. Subsequently feto-placental signals play a major role in regulation of maternal metabolism. This chapter describes the maternal adaptation to pregnancy and the role of the placenta in nutrient transfer to the fetus.rnrn

Drugs for Pregnant and Lactating Women

The study of medication use in pregnancy is one of the least developed and most neglected areas of clinical pharmacology and drug research. Although pregnancy is widely regarded as a special population due to both the unique maternal physiology and the vulnerability of the developing fetus, researchers and pharmaceutical companies have been reticent to evaluate optimal modalities of treatment for this group. The issue is compounded by the enormous number of medications women are exposed to during pregnancy. Epidemiological surveys indicate nearly two thirds of all pregnant women use four to five drugs during pregnancy throught delivery. Women with medical conditions such as epilepsy, diabetes, and hypertension must continue therapy while pregnant. In some cases, due to a justified or unjustified concern for the developing fetus, the medication prescribed is either withheld, inadequate to treat the maternal condition, or not monitored closely enough as pregnancy progresses for needed adjusments in dosing. The result is a double negative, that of fetal exposure without maternal of fetal benefit. The lack of Food and Drug Administration obstetric labeling and the near universal off-label use of drugs are the direct results of the paucity of research and clinical trials in this special population. The public concern stems from the use of drugs in pregnancy based on an empiric approach rather than a scientific basis, and does not take into account the many alterations in pregnancy.

The discovery of the additional significance of mother?s breast and chest to the infant comes from studies of Swedish researchers who have shown that a normal infant, when dried, placed on the mother?s chest, and covered with a light blanket, will warm or maintain body temperature as well as an infant warmed with elaborate, high-tech heating devices. The same researchers found that, when infants are skin-to-skin with their mothers for the first 90 minutes after birth, they hardly cry at all compared with infants who are dried, wrapped in a towel, and placed in a bassinet. In additional, the researchers demonstrated that if a newborn is left quietly on the mother?s abdomen after birth he or she will, after about 30 minutes, gradually crawl up to the mother?s breast, find the nipple, self-attach, and start to suckle on his her own.rnrnrnIt would appear that appear that each of these features-the crawling ability of the infant, the absence of crying when skin-to-skin with the mother, and the warming capabilities of the mother?s chest-evolved genetically more than 400,000 years ago to help preserve the infant?s life.rn
